This form is used in Solano County civil cases when a defendant has been personally served with a summons and complaint for money damages only, fails to appear or file a response within the time allowed by law, and the plaintiff requests entry of default. After the clerk enters default and any credits are acknowledged, the clerk may enter judgment for the principal amount, interest, costs, and attorney’s fees (pursuant to the court’s adopted fee schedule).
