PETITION FOR MINNESOTA NO-FAULT ARBITRATION. This form is used to initiate the process for resolving disputes arising from no-fault insurance policies in Minnesota. The form is used by Claimants who want to resolve disputes through the Minnesota No-Fault Rules of Procedure administered by the AAA. To begin the process, the Claimant must file a completed Petition for No-Fault Arbitration, along with supporting documents, a denial/discontinuation letter from the insurance company, an itemization of the claim, and a $40 filing fee payable to the American Arbitration Association. The itemization should detail what the Claimant is claiming for arbitration and include the names of medical providers, the names of employers, dates of service or loss, and the amounts claimed for each. The supporting documents may include billing summaries, wage stubs, market value comparisons, and any other documentation that supports the Claimant's claim. Once the filing fee and required documents are submitted to the AAA, the arbitration process can begin. The Minnesota No-Fault Arbitration Rules of Procedure administered by the AAA govern the arbitration process, and the arbitrator's decision is binding.
