SMALL CLAIMS NOTE FOR MOTION DOCKET. This form is used in Kitsap County District Court to formally schedule a hearing for a motion in a small claims case. This form notifies the court and opposing parties of the motion being filed and provides the date, time, and courtroom where the hearing will take place. Before filing, the requesting party must confirm availability with both the opposing party (or their attorney) and court staff. Once a hearing date is confirmed, the party must file both the written motion and this Note for Motion Docket form with the court within one judicial day. The documents must also be properly served on all opposing parties at least five days before the hearing (or eight days if served by mail). A Declaration of Service must then be filed with the court to prove that all parties were properly notified.
