MC-520 - MOTION FOR ORDER TO BE FORWARDED TO THE FEDERAL GOVERNMENT TO RELIEVE ME FROM NOT BEING ABLE TO POSSESS FIREARMS. This form is used by individuals in Alaska who are seeking legal relief from a federal prohibition on firearm and ammunition possession. This restriction applies to individuals who have previously been involuntarily committed to a mental health facility for 30 days or more, or who have been found by a court to be mentally ill or mentally incompetent. By completing this form, the individual formally requests that the court issue an order lifting the firearm disability and forward it to the Alaska Department of Public Safety.
