L-3016 - ACTIVITIES OF DAILY LIVING (ADL) ASSESSMENT OF THE PROPOSED CONSERVATEE. This is a Orange County Superior Court form used in California conservatorship proceedings to evaluate a proposed conservatee’s ability to manage daily personal, medical, and financial tasks. The form gathers detailed information about physical functioning, cognitive limitations, instrumental activities of daily living (IADLs), financial management skills, safety concerns, abuse risks, support systems, and recommended services.
