LSC-CIV-050 - NAME CHANGE CRIMINAL HISTORY ASSESSMENT. This form is used in the Superior Court of California, County of Lassen, as part of a name change proceeding. Required under California Code of Civil Procedure § 1279.5, the form collects identifying information from the petitioner and is used by the Lassen County Sheriff’s Department to conduct a criminal history review. The assessment determines whether the petitioner is a registered sex offender, under the jurisdiction of the Department of Corrections, or otherwise identifiable in criminal databases.
