ORDER FOR REVIVOR. This form is used in the Court of Common Pleas, Hamilton County, Ohio to revive a dormant judgment. A judgment becomes dormant when no action, such as execution or collection, has been taken on it for a period of five years. This form is used when the Judgment Creditor (the party to whom the money is owed) files a motion to have the judgment revived, thereby reactivating the debt and allowing collection efforts to resume. The Order for Revivor states that the judgment will be revived, effective from the date of the court's order. The form includes a summons that is sent to the Defendant/Judgment Debtor (the party who owes the money), notifying them of the motion to revive the judgment. The Defendant has the opportunity to respond within 28 days. If no objection is made, the judgment is automatically revived.
