ATTORNEY NOTICE OF APPEARANCE. This form is used by attorneys appearing before the Office of Administrative Law Judges (OALJ), U.S. Department of Labor to officially enter representation in a case. Under 29 C.F.R. § 18.22(a), attorneys must attest that they are licensed and in good standing before the highest court of a U.S. state, territory, or the District of Columbia, and that no disciplinary proceedings are pending against them. The form requires identification of the client (e.g., claimant, respondent, amicus), the attorney’s signature, address, phone, email, jurisdiction of licensure, and license number. Filing the notice also acknowledges obligations under OALJ rules, including prompt disclosure of any future disciplinary actions, compliance with rules of professional conduct, and the duty to act diligently and ethically in the proceeding.
