REQUEST FOR ISSUANCE OF ALIAS/PLURIES SUMMONS.This form is used in the County Court of the Seventh Judicial Circuit in and for St. Johns County, Florida, when an original summons was not successfully served on a defendant. This form allows a plaintiff to formally ask the Clerk of Court to issue a new summons—either an alias summons (second attempt) or a pluries summons (subsequent attempts).
