L-0006 - REQUEST FOR RECORD SEARCH. This form is used to request a name-based court record search in the Superior Court of California, County of Orange. This optional form allows individuals to search specified case types—such as unlimited civil, limited civil, family law, probate, small claims, criminal, or traffic—within designated Justice Centers. The requestor must provide identifying information, including full name, date of birth (if known), driver’s license number (if known), and the years to be searched. A record search fee of $15.00 applies per name and per case type searched. Payment may be made by check payable to the Clerk of the Court or by authorized credit card.
