CCR A-160 - PETITION FOR CHANGE OF NAME. This form is used to petition for a legal name change in Fairfax Circuit Court, Virginia, in accordance with Virginia Code §8.01-217. The petitioner must provide their current legal name, desired new name, address, date and place of birth, and parents' names. Additionally, the petitioner must disclose whether they have previously changed their name, have any felony convictions, are incarcerated, are on probation, or are required to register as a sex offender. The petition must be signed under oath and notarized before submission. If the court approves the request, a Final Order of Name Change is issued, officially changing the petitioner’s name.
