This is a Kern County Superior Court Family Law form used after a judgment of dissolution, legal separation, or nullity has been entered. The form allows a petitioner or respondent to request a court order confirming that their former name was not restored in the judgment and that they have retained their marital surname since entry of judgment. The court’s order formally acknowledges continued use of the marital surname as of the filing date.
