WAIVER OF NOTICE OF HEARING ON PETITION IN ACCORDANCE WITH D.C. CODE, SEC. 21-2032. This form from the Superior Court of the District of Columbia Probate Division is used in guardianship or conservatorship proceedings involving an adult. The form allows a person who is entitled to notice of a scheduled hearing—such as a family member or interested party—to formally waive that right. By signing the waiver, the individual acknowledges that they have received a copy of the petition to be heard, are aware of the hearing’s date, time, and location, and understand their right to respond within ten days. Despite having that right, they choose not to receive further notice.
