MOTION TO ORDER or WAIVE MEDIATION and DECLARATION IN SUPPORT. This form is used in the Circuit Court of the State of Oregon by either the petitioner or respondent in a domestic relations case involving contested custody or parenting time. The purpose of the form is to request that the court either require the parties to participate in mediation or excuse them from mediation requirements. Mediation is often used to help parties resolve disputes regarding parenting plans without the need for a full court hearing.
