L-0703 - RESPONSE BY ELDERCARING COORDINATOR. This form is used in Orange County eldercaring coordination cases and is completed by an appointed Eldercaring Coordinator to accept or decline the appointment. The coordinator must confirm whether they meet the qualifications recommended by the Association for Conflict Resolution Task Force on Eldercaring Coordination or explain their qualifications if selected by mutual consent. The form also requires the coordinator to disclose any conflicts of interest and acknowledge understanding of their role, authority, and responsibilities under the court’s Order Referring Parties to Eldercaring Coordinator.
