DEBTOR’S ELECTRONIC BANKRUPTCY NOTICING REQUEST (DeBN). This form is used in the U.S. Bankruptcy Court for the Western District of Wisconsin by debtors who want to manage how they receive court-generated notices and orders during their bankruptcy case. Debtors can use the form for three main purposes: to request initial enrollment in the DeBN program to receive notices via email rather than U.S. mail, to update their DeBN account information such as a new email address or to reactivate a previously disabled account, or to deactivate their DeBN account and return to receiving notices by regular mail. The form explains that DeBN participation is voluntary and applies only to notices issued by the court—not to documents from trustees or creditors. Debtors are also informed of the consequences of an undeliverable email and are required to submit valid identification with the form.
