FORM 1007-1F - PAYMENT ADVICES CERTIFICATION. This form is used in the United States Bankruptcy Court for the Northern District of Oklahoma by individual debtors to comply with the requirements of 11 U.S.C. § 521(a)(1)(B)(iv). This section of the Bankruptcy Code mandates that debtors provide the court with copies of all payment advices or other evidence of income received from their employer(s) within the 60 days prior to filing a bankruptcy petition. This form is required for each debtor in a joint case and must be filed separately. The debtor uses this form to certify one of three things: (1) that all required payment advices have been filed or attached; (2) that the debtor received such documents but has not yet submitted all of them, with the understanding that failure to do so within 45 days of the petition date may result in dismissal of the case; or (3) that no payment advices were received during the 60-day period.
