L-0211 - REQUEST FOR RELEASE OF CONFIDENTIAL REPORT. This form is used in Orange County guardianship and conservatorship proceedings to request access to a confidential court investigation report . Pursuant to Probate Code §§ 1513 and 1826, investigation reports are confidential and may be released only to authorized individuals. The requesting party must identify their relationship to the case and, if not already entitled to receive the report, explain the reason for the request.
