L-0984b - DECLARATION OF PHYSICIAN OR QUALIFIED LICENSED PSYCHOLOGIST CONSERVATORSHIP RE-EVALUATION. This form is used in Orange County conservatorship proceedings and completed by a licensed physician or qualified psychologist to assist the Court in determining whether a conservatorship should continue. The evaluator must provide the conservatee’s identifying information, prior diagnosis, current evaluation date, and detailed responses regarding (1) the existence of a mental disorder, (2) the individual’s ability to provide for basic needs such as food, clothing, and shelter, and (3) whether the person is incapable or unwilling to accept voluntary treatment.
