FL14077 - PROOF OF SERVICE OF NOTICE OF EX-PARTE APPLICATION FOR EMERGENCY COURT ORDERS (NON-DOMESTIC VIOLENCE). This is a Yuba County Superior Court family law form used to document that proper notice was given before an ex parte hearing. This form must be filed by noon on the day of the ex parte hearing and must be completed by a person who is not a party to the case. The declarant confirms how and when notice was provided (e.g., phone, voicemail, in person, written, fax, text, email, or social media), states the nature of the requested temporary emergency orders (such as custody or other relief), and indicates whether the opposing party intends to appear.
