FAM FCS 021 - CHILD CUSTODY EVALUATION PARENT QUESTIONNAIRE. This is a Los Angeles Superior Court Family Court Services form used in child custody evaluation cases. When the court orders a custody evaluation, each parent must complete a questionnaire and return it to the Child Custody Evaluation Office within 10 days in the attached envelope. A signed Authorization for Release of Information is also required. The questionnaire gathers comprehensive information about each parent’s identifying details, employment, income, medical and counseling history, criminal record, marital history, children’s living arrangements, schooling, medical and counseling history, DCFS involvement, history of domestic violence, and proposed custody/visitation plans. It may also require information about any stepparent or adult living in the home. The questionnaire is important for introducing you and your situation to the evaluator assigned to your family. When returned, you will be contacted by an evaluator regarding an office appointment and plans to visit your home and interview your children.
