OCSD5 - ORANGE COUNTY SHERIFF’S DEPARTMENT EVICTION INSTRUCTIONS. This form is used to provide required information to the Orange County Sheriff’s Department when enforcing a Writ of Possession in an eviction case. This form accompanies the writ and authorizes the Sheriff to serve a 5-day Notice to Vacate and remove occupants pursuant to Code of Civil Procedure §§ 715.010–715.050. The judgment creditor must identify the property address, confirm whether a prejudgment claim of right to possession was served, specify whether the property is real or personal property, and provide a contact person who will be present at the eviction.
