RI-ADR011 - POST-MEDIATION SURVEY. This is a Riverside County Superior Court form used to gather feedback from participants following court-ordered mediation. All mediation participants—including parties, attorneys, insurance representatives, and non-parties—may complete the survey. The form collects information about case type, settlement outcome, number of sessions, cost savings, court time avoided, and overall satisfaction with the mediation process and mediator performance. It also includes a detailed rating scale addressing fairness, communication, mediator effectiveness, fees, and whether the participant would recommend mediation in the future. Responses are used for statistical reporting and program improvement, and individual evaluations are not shared with trial judges and non-ADR court staff.
